The contemporary discussion of chronic rheumatic pain has begun to move beyond a narrow inflammatory model. Recent reporting from EULAR 2026 emphasized that a substantial proportion of patients continue to experience severe or unacceptable pain years after diagnosis, even in the context of anti-inflammatory treatment, and proposed several important contributors to this persistent burden, including nociplastic pain, poor sleep, psychological distress, and kinesiophobia. This shift is important because it recognizes that pain persistence cannot always be explained by active inflammation alone.  Yet the findings may support an even deeper interpretation.
